Eligibility
All employees receive a card with Sponsor name and number listed on it. This card can be used as often as necessary.

Benefits
We have a network of participation Ophthalmologists, Optometrists
and Opticians from which to choose to obtain vision care benefits.

Lenticular –the participating provider will charge his UCR 25%

Lens Options
The participation provider will charge you up to the wholesale cost plus 50% on each Option selected, or their UCR fee.
Standard Lens Photo chromatic, Scratch resistant, Fashion or tints, Progressive or Options Include: No-Line multi-focal, Anti-reflective, Polycarbonate and Prescription Sunglasses.

Contact Lenses
The participation provider will charge his UCR fee less 25% for contact lens exam.

Frames
The participating provider will charge up to the wholesale cost for
frames plus 50% or their UCR if less. This averages 50%-60% discount from the retail pricing.

Exclusions
The following services and materials are not covered under this
program: Medical or Surgical treatment of the eyes / Drugs or medications / Non-prescription lenses / Examinations or Materials not listed as a covered service, Services or materials provided by Federal, State, and Local Government or Workers Compensation / Low vision aids.

Lasik
There is a 25%-30% discount from retail pricing through NVA.
